In her role as Registered Nurse Integrated Care Coordinator, Charlene exemplifies excellence by serving as a vital link between patients and essential healthcare services. She guides new clients through available resources, conducts thorough health assessments, and ensures timely access to necessary care. Moreover, Charlene plays a crucial role in addressing social determinants of health, such as transportation and housing needs, to improve overall well-being. 

Beyond her primary responsibilities, Charlene’s willingness to take on additional roles, such as a Community Health Worker and a mentor for new Integrated Care Coordinators, underscores her leadership qualities and dedication to professional growth. Her proactive involvement in mentoring and training reflects her commitment to fostering a culture of excellence and collaboration within the healthcare community.

In an interview with Louisville Business First, Charlene said being a Health Care Hero means maintaining a passion for nursing and continuing to put patients and clients first, even during difficult times. Her goal for 2024 is to continue that passion and keep training others in their nursing careers. “Above all else, I want to teach my clients, mostly children and teens, that seeing and talking to a nurse doesn’t have to be scary,” she said.

Charlene’s recognition as a Rising Star Health Care Hero is a testament to her exceptional achievements and unwavering dedication to the well-being of her clients and colleagues.
